Recognizing Program Enhancements

Unique to Community Care is our service delivery and programming for Housing, specifically our focus on homelessness prevention. The roles involved in this work are diverse, from Trustee Administrators and Caseworkers to Intake staff and Rapid Rehousing workers, all are important pieces of a complex puzzle.

Within the Housing Help program, the Consortium established in 2024 has yielded some truly wonderful and positive results for the homelessness prevention work. For example, as you’ll read in the 2024 Annual Report, in just one area of the program called Eviction & Loss Prevention, there were 87 clients rehoused, with 85% of clients remaining housed after 6 months. The cost estimate of the economic benefits of households avoiding eviction and maintaining their housing is significant. The savings are for the individual as well as for the Government and other resources.

The Consortium team includes our partners at ProjectSHARE, The Hope Centre and Community Care of West Niagara. Together, we are able to help support more clients regionally, while also tackling some of the inherent systemic issues that working in isolation cannot address.
